1. Nature of the Program

• This program provides evidence-based lifestyle medicine interventions focused on the prevention, management, and reversal of chronic diseases through nutrition, stress reduction, physical activity, sleep optimization, and other lifestyle practices.
• Participation in this program does not replace or eliminate the need for a primary healthcare provider, and it does not in itself create a doctor-patient relationship with PrimeLife360 Wellness, its founders, or affiliates.

2. Medical Collaboration

• I understand that I should continue regular follow-up with my physician or licensed healthcare provider during my participation.
• I agree not to discontinue or modify prescribed treatments without consulting my healthcare provider.
• In the event of a medical emergency, I agree to call my local emergency number immediately and not rely on this program for urgent medical assistance.

3. Technology & Privacy

• This program is conducted virtually. I understand I must have access to a reliable internet connection, video/audio capabilities, and basic technical skills to participate.
• I acknowledge that absolute confidentiality cannot be guaranteed in virtual environments, though PrimeLife360 Wellness will make reasonable efforts to protect my information.
• I consent to the collection, storage, and use of my data in accordance with the Privacy Policy.
• Unauthorized recording, reproduction, or distribution of program sessions, materials, or discussions is strictly prohibited.

4. Intellectual Property

• All program content, including but not limited to videos, guides, presentations, and tools, is the intellectual property of PrimeLife360 Wellness and is provided for personal use only.
• I agree not to copy, share, reproduce, resell, or distribute any program content without prior written consent.

5. Participant Responsibility

• I understand that the effectiveness of the program depends on my active participation and commitment to applying the lifestyle medicine principles provided.
• I acknowledge that while lifestyle medicine is safe, natural, and effective for most individuals, outcomes will vary depending on my health condition, adherence, and personal circumstances.
• I agree to participate respectfully in group sessions and understand that disruptive or inappropriate behavior may result in removal from the program without refund.

6. Payment, Refunds & Transfers

• All payments are final. I acknowledge that there are no refunds once enrolled.
• If enrollment coincides with program capacity limits, I may be offered a spot in the next available group or an option to apply my payment to other services.
• Access to program content is non-transferable.

7. Limitation of Liability

• I recognize that lifestyle medicine practices recommended in this program are safe and beneficial when appropriately applied.
• I understand that my results depend on my individual effort and adherence, and that PrimeLife360 Wellness does not guarantee specific outcomes.
• I release PrimeLife360 Wellness, its employees, and affiliates from any liability for outcomes related to my participation, except in cases of willful misconduct or gross negligence.

8. Consent & Agreement
